fix insert/stream crashes with segfault if more than one insertion face is used

Status :
(31 August 2011): Implemented a possible remedy that will be part of the next release 1.4.2
(8 Sept 2011): Fix released in 1.4.2, waiting for user feedback if the bug has vanished
(12 Sept 2011): Remaining issue, hopefully to be fixed by the attached files
